A multi-site women’s health practice was processing 1,200 medical record requests daily. The internal team of six abstractors was three weeks behind. A prior AI consultant had delivered a proof of concept that performed at 60% accuracy. Hi-Tek was brought in to either fix it or rebuild it.
Rebuilding a medical record abstraction pipeline that actually ships to production
The prior proof of concept ran on a generic prompt against a single model with no evaluation framework. Hi-Tek scrapped the prototype, ran an evaluation against a 200-record gold set across three models (Claude, GPT-4, Azure OpenAI), and selected Claude based on accuracy and HIPAA-aligned controls. The system was rebuilt with structured outputs, confidence scoring, and a human review queue for low-confidence cases.
Production cutover was at day 90. By day 120 the agent was processing all 1,200 daily records, the human review queue surfaced approximately 12% of cases for review, and the abstraction team had transitioned from six full-time abstractors to two reviewers handling the queue plus a small set of high-complexity manual abstractions. The pipeline has been running continuously since.
